We care about patient privacy and are committed to protecting your health information. This Patient Privacy Policy (posted here and in the clinic lobby) describes how your medical information may be used and disclosed and how you can access this information. Please review it carefully.
YOUR RIGHTS
- You have a right to a copy of your medical record. Colorado law states that we must provide a copy within 10 days of the request for former patients and within 30 days for current patients.
- If you disagree with the health information in your file, you may submit a statement of disagreement letting us know what you believe to be incorrect or incomplete. We will respond in writing within 45 days of your request. We have the right to say “no” to the changes that you suggest, but we will explain in our response why your claim was denied.
- You can designate your preferred method of communication (phone, text, email, etc) and we will do our best to accommodate your request.
- You can ask for a copy of this policy at any time or view it at www.thealphacenter.org/privacy.
- If you feel we have violated this policy and put your identifying health information at risk, you can file a complaint with the Colorado Attorney General. We will not retaliate against you for doing so.
YOUR CHOICES
- You can share your preferences with us about who and what is shared with your family, close friends, or others involved in your care. Without explicit consent, no information will be shared with them unless you are unconscious or there is a serious or imminent threat to your health or safety. All reasonable measures will be taken to safeguard your sensitive data.
- We will not share your information with third parties without your written permission.
- We may contact you for fundraising efforts, but you can request that we remove you from future fundraising outreach.
OUR USES AND DISCLOSURES
- With your written consent, we can share your health information with other professionals who are treating you.
- We can use and share your health information to run our practice, improve your care, and contact you when necessary. We will also share information about you if required by applicable state or federal laws, such as workers’ compensation, law enforcement, and other government requests.
- We can share health information about you in response to a court or administrative order or a subpoena within the confines of Colorado law.
- For uses beyond these, we will ordinarily obtain your written authorization.
- If you are a Banner patient, your test results may appear in your patient portal as Banner’s labs process our testing.
OUR RESPONSIBILITIES
- We abide by all applicable state and federal laws to maintain the privacy and security of your health information.
- We will follow all applicable state and federal laws if a breach occurs that may have compromised the privacy and security of your health information.
- We will not use or share your information other than as described here unless you tell us we can in writing. If you tell us we can, you may change your mind at any time by writing to us to rescind your permission.
CHANGES TO THE TERMS OF THIS PATIENT PRIVACY POLICY
We can change the terms of this policy, which will apply to all information we have about you. The new policy will be available upon request in our office and on this page.
